When a packet is created on a device, the IP protocol determines if the destination IP is on the local or remote network. If the destination is unknown the host sends the packet to the gateway. If the destination is local, the host forwards the packet according the destination MAC Address. A router that receives a frame will de-encapsulate it, check the destination IP in the packet, discard the frame and create a new one with the next hop hardware information. The IP information will not be modified. In case that NAT is being used to exit a network, the router will indeed change the source IP of a packet.
