TTL was initially a value in seconds and so was intended to decrement based on how long the packet was in the router. Over time it has become the norm for a router to decrement the TTL by 1 when sending it out because packets rarely spend even a full second being processed. There are scenarios where it can drop by more than that though they are rare. IPv6 renamed the TTL to Hop Limit to better reflect it's actual usage.
