All of these are security incidents (It might seem like "scanning" is not a security incident, but it is a recon attack that precedes other more serious attacks). I disagree with the answer: Malicious code in and of itself is not an incident. There's plenty of malicious code on the internet but its not an INCIDENT to my network unless the malicious code is found in my network. Just and observation of what the question is actually asking.
